New version of Authentication Application Form of the People's Republic of China will be used starting from January 1st, 2018.
Please be advised that ALL authentication applicants are required to use the new Form. Please fill out the form truthfully, completely and clearly in English.
The form of previous version will not be accepted starting from January 1st, 2018.
